Proxy Preferences

As a Provident Credit Union member, it is important to submit a “voting proxy” form. This enables your Credit Union’s Board of Directors to represent you at membership meetings if you are unable to attend. To ensure the continued successful and consistent operation of your Credit Union, please sign and return the enclosed proxy replacement card at your earliest convenience.

I, the undersigned member of Provident Credit Union, do hereby constitute and appoint a committee composed of the Chairman, Vice Chairman and President as my true and lawful proxy, with full power of substitution and revocation, to attend any and all regular or special meetings of the members of said Credit Union to be held between the date hereon and three years hence, or any continuation or adjournment thereof, with full power to vote and to act for me at said meetings with all the powers the undersigned would possess if personally present except as indicated below:
