Credit Card Replacement

Provident credit card holders have the option to report their credit card as damaged, lost or stolen via online or mobile banking. Members must be enrolled in online banking to access this service.

How to Report Damaged, Lost or Stolen Credit Card

  1. Credit card holders must first be enrolled in online banking to access this service. For more information on how to enroll in Online Banking, please click here.
  2. Once signed-on to online banking, select Card Controls, then select Credit Card Controls from the Settings menu.
  3. Select the credit card that is damaged, lost, or stolen from the drop down menu.
  4. Scroll down to the “Damaged, Lost, or Stolen” section of the Credit Card Controls page and turn the toggle switch on.
  5. Select either the “Card is Damaged” or “Card is Lost or Stolen” options.
 

Card is Damaged

  1. For standard 7-10 business day delivery, just click the “Report and Re-Order” button.
  2. For expedited 1-2 business day delivery, select the "Express Delivery" checkbox and then click the “Report and Re-Order” button. There is a $25 fee for expedited delivery.
 

​Card is Lost or Stolen

  1. Input date credit card went missing.
  2. Select the State and Place your card was lost from the drop down menus.
  3. Select the security check box that applies.
  4. If you check the “I suspect or know this card has been stolen” checkbox then a list of your most recent transactions will show.
  5. Review your transactions for possible fraud. Click “Show 5 More” to display more transactions.
  6. For standard 7-10 business day delivery, click the “Report and Re-Order” button. 
  7. For expedited 1-2 business day delivery, select the "Express Delivery" checkbox and then click the “Report and Re-Order” button. There is a $25 fee for expedited delivery.

NOTE: Please make sure the correct card is selected, as reporting a card damage, lost or stolen cannot be undone and will permanently disable all of your existing cards with the same card number. Any pending charges or scheduled payments to this card will therefore fail.
