Use this Direct Deposit Set Up to set or change your options for either Direct Deposit or Payroll Deduction from your paycheck or benefits check.
Use this Automatic Loan Payment Set-Up to set or change your options for Automatic Payment to your Provident Credit Union loan from another account at either Provident Credit Union or another financial institution.
If you are receiving a periodic federal check (such as monthly Social Security benefits), you can apply online for Direct Deposit through Go Direct, a U.S. Government Web site.

These forms can be completed either before or after printing it. Sign your form and mail to:
