Use this Direct Deposit Set Up to set or change your options for either Direct Deposit or Payroll Deduction from your paycheck or benefits check.
Use this Automatic Transfer Set-Up to set or change your options for Automatic Transfer to your Provident Credit Union account from another account at either Provident Credit Union or another financial institution.
If you are receiving a periodic federal check (such as monthly Social Security benefits), you can apply online for Direct Deposit through Go Direct, a U.S. Government Web site.

This form can be completed either before or after printing it. Sign your form and mail to:
